Note: All students attending a Berkeley Unified School District school in the 5th grade will receive middle school enrollment papers in the mail in early January, 2009. Students attending 5th grade elsewhere should submit paperwork in January to enroll for the 2009/10 school year.

2009/10 Enrollment

Middle School Nights (Noches Informativas en Espanol) -- Each middle school will host an information night in January where parents will have a chance to visit the school, meet teachers and the principal, and discuss curriculum. Information nights are a great opportunity to ask questions! All new middle school families are encouraged to attend -- both those new to the district and those matriculating from a Berkeley elementary school. All nights begin at 7 pm. Enrollment Procedures

Parents of students who are entering Berkeley's middle schools (6th through 8th grade) must submit a Parent Preference Form listing school preferences, along with the student's birth certificate and three original proofs of Berkeley residency. Students who are already attending a Berkeley Unified elementary school need only submit the form; proofs and a birth certificate are not required unless you have moved.

Proofs of Berkeley residency must be recent and have the address and parent/guardian's name. Acceptable proofs include:

PG&E bill

EBMUD bill

Garbage

Telephone bill (not cell phone)

Cable bill

Bank Statement

Paycheck stub

Drivers License

Action letter from Social Services

Berkeley Housing Authority documentation

NOTE: At least one proof of residency must be a utility bill (PG & E, EBMUD, Telephone, Garbage, Cable or Land-line phone).

Parents may request any middle school in the District, however priority will be given to families who live in the school's assignment zone. Longfellow is a district-wide school, so students from either zone can request to attend Longfellow by submitting the Longfellow Preference Form below.
